Birth certificate (Australian)
A birth certificate can refer to either an original document or a certified copy issued by an Australian State or Territory. When using a birth certificate as a form of ID in the USI registry system, please be aware that:
- state refers to the state or territory in which the birth was registered
- 'extracts' of birth certificates are not accepted
- the registration number is a unique number. Its location differs on each state and territory certificate
- date of registration is the date that the birth was registered
- year of registration is the year the birth was registered
- date printed is the date the birth certificate was printed. Not all birth certificates have this information
- certificate number must be the same as that shown on the birth certificate.

NT Birth Certificates issued from 1870 onwards can be verified through the DVS.
Note:
certificate numbers are required on all certificates issued on or after 12 July 1999.
there are three types of registration numbers in Northern Territory birth certificates.
1. Pre 1987 registration numbers have a prefix of NT, NN, NNA, NCA, CA etc. followed by a number in the form of 123/45. Only enter the numbers after the /.
1987 – 1999 registration numbers have a prefix B or D followed by a number in the form of 2000/00001.
Only enter the numbers after the /
after 1999 the registration is a straight 8 digit number.
Certain Northern Territory Birth Certificate need to use the following formatting:
enter registration numbers for certificates issued in the 1940’s (i.e. page 121 book 1) as 1 for verification
enter registration numbers for certificates issued in the 1950’s (i.e. 93/93/21) should be entered as 21 for verification
enter registration numbers for certificates issued in the 1960’s (i.e. 136 book 40) as 40 for verification
enter registration numbers for certificates issued in the 1970’s (i.e. N.T. 232.70) as 70 for verification
enter registration numbers for certificates issued in the 1970’s (i.e. 54/35) as 35 for verification
enter registration numbers for certificates issued in the 1970’s (i.e775/155/18) as 18 for verification
enter registration number for certificates issued in the 1980’s (i.e. 1631/3234/17B) as 17 for verification. Ensure any letters are excluded.
Birth Certificate NT1
The registration number would normally appear on the top right hand corner (where the number 28 appears). However, the majority of these certificates were originally handwritten and the registration number could appear in different sections of the certificate (ie next to the date of birth column on the left hand side of the certificate or at the bottom of the certificate next to the Registrars/Deputy Registrars signature).

Queensland

QLD birth certificates issued from 1 January 1974 onwards can be verified through the DVS.
Omit the year and the "/" when submitting a registration number from Queensland. This prevents validation errors.
The registration number will be validated from birth certificates where the date of registration is greater or equal to 1 July 1996.
For all certificates from 1 July 1974 to 1 July 1996 the query terms validated against are:
family name
given names
date of event
registration date.
For all certificates after 1 July 1996, the query terms validated against are:
family name
given names
date of event
registration date
registration number.

Since 2009, the WA government has been digitising certificates for births registered before 1974. Digitised certificates will look different to paper-based certificates.
Birth certificate for 1930 – 1950
Note:
the registration number you enter into the DVS should be 7 digits long. It should comprise the district number followed by registration number. The district number should make up the first 2 digits, and the registration should make up the remaining 5 digits
if the district number is less than 2 digits long, or the registration number is fewer than 5 digits long, place additional 0’s in front to make up the remaining characters ie if the District Number is 1 and the registration number is 123, then you should enter the 7 digit number into the DVS as 0100123.

Birth Certificate for 1961 – 1983 – Alternative Format Certificate
Note:
Some births may have been registered in this period with less information than the usual format birth registration. As a consequence, these birth certificates will contain less detail and do not conform to the usual Registration District numbering conventions.
These birth certificates can be identified by the format shown in the sample below.
The District Number for these certificates is 99, despite the place of registration showing “Perth”.
Therefore, in the sample below, the Registration number is formed by adding the District Number of 99 and the Registration Number (consisting of 5 digits) of 00048. The 7 digit number to enter becomes 9900048

Re-format case 1
Where registration number appears like 123456X/YYYY append a leading zero to first six numbers to make up the required 7 digit registration number i.e. 0123456
Re-format case 2
Where registration number appears like 1234567X/YYYY make up the required 7 digit registration number i.e. 1234567
Re-format case 3
Where registration number appears like 1/ 234/56/X make up the required 7 digit registration number i.e. 0100234
The single digit before the first slash first needs to be padded to 2 digits with a leading zero
The numeric value between the first and second slash (“/”) needs to be padded to 5 digits with leading zeroes
The two values needs to be strung together

Where registration number appears like 12/ 345/67/X make up the required 7 digit registration number i.e. 1200345
The numeric value between the first and second slash (“/”) needs to be padded to 5 digits with leading zeroes
The first two digit number needs to prefix the 5 digit number above to make up the 7 digit numeric number

All other WA Birth Certificates will show a registration number and a DISTRICT name. eg 456/63 in District of PERTH
Look up the WA Registration District Code listing for DISTRICT name specified on the Birth Certificate to obtain the associated 2 digit code for that district. PERTH equates to 01 ** See note 1
Pad the registration number out to 5 digits eg 456 becomes 00456 - ignoring the /63 part of what appears on the birth certificate
Join the 2 digit number to the front of the 5 digit number to make up the required 7 digit number. i.e. 01 & 00456 becomes 0100456

Sometimes the District Name appears in the section of the WA birth certificate which refers to when and where the registration took place.
If not found in the listing under the column headed “District Name” search through the column headed “Name of the town in the district where the birth was registered”. Eg. Where WA certificate says registered in Narrogin its corresponding District code is 67.
